Chronic disease management at YourSkin Clinic

New or repeat prescriptions for medication

Many Australians live with one or more chronic conditions. Our GP is experienced in helping patients manage ongoing illness effectively, with the goal of reducing symptoms, preventing complications, and improving quality of life.

What is a Chronic Disease Management Plan?

If you have a chronic condition that has been present for at least six months or is likely to be present for that time, your GP can prepare a GP Management Plan (GPMP) for you. This is a written plan that outlines your health goals, the treatments and services you need, and the actions you will take to manage your condition. It is reviewed and updated annually. A Team Care Arrangement (TCA) can also be prepared alongside a GPMP, which allows you to access Medicare-rebated visits with allied health professionals such as physiotherapists, dietitians, podiatrists, and exercise physiologists. Up to five allied health visits per calendar year are covered under this arrangement.

Common chronic conditions we manage include:

Diabetes (Type 1 and Type 2)

Diabetes management involves regular monitoring of blood glucose levels, HbA1c blood tests, kidney and eye checks, foot care, and medication or insulin management. Your GP will coordinate your care across relevant specialists including endocrinologists, ophthalmologists, and podiatrists. Lifestyle modification including diet and exercise is a cornerstone of management and our GPs can refer you to a dietitian and diabetes educator under a care plan.

High blood pressure (Hypertension)

Most people with high blood pressure have no symptoms, which is why regular checks are essential. Uncontrolled high blood pressure significantly increases the risk of heart attack, stroke, kidney disease, and vision loss. Management includes lifestyle advice, monitoring, and medication where appropriate. Blood pressure targets will be discussed and agreed with you individually.

High cholesterol

Elevated cholesterol increases the risk of heart disease and stroke. Management depends on your overall cardiovascular risk profile, not your cholesterol number alone. Your GP will assess your total risk and discuss whether lifestyle changes, medication, or both are appropriate for you.

Asthma

Asthma management focuses on minimising symptoms, reducing flare-ups, and ensuring you have a current written Asthma Action Plan. Your GP will review your inhaler technique, ensure you are on the most appropriate medication regimen, and identify and address triggers where possible.

Chronic Obstructive Pulmonary Disease (COPD)

COPD is a progressive lung condition most commonly caused by smoking. Management includes smoking cessation support, appropriate inhaler therapy, pulmonary rehabilitation referral, and monitoring for complications. Annual influenza vaccination and five-yearly pneumococcal vaccination are strongly recommended for all patients with COPD.

Thyroid conditions

Both underactive (hypothyroidism) and overactive (hyperthyroidism) thyroid conditions require ongoing monitoring with blood tests and medication adjustment over time. Your GP will manage your thyroid function and refer to an endocrinologist where needed.

Heart disease

Patients with a history of heart attack, angina, heart failure, or other cardiac conditions require regular review of symptoms, medications, blood pressure, cholesterol, and lifestyle factors. Your GP coordinates your care alongside your cardiologist and ensures your risk factors are well controlled.

Arthritis

Both osteoarthritis and inflammatory arthritis (including rheumatoid arthritis) are managed in general practice. Management may include pain relief, physiotherapy referral, anti-inflammatory medications, and for inflammatory arthritis, referral to a rheumatologist for specialist review and disease-modifying therapy.

Many patients require ongoing prescriptions for long-term conditions. We can provide continuing prescriptions for established medications as part of your ongoing care.

A brief consultation is generally required to renew a prescription. This ensures:

  • Your medication remains appropriate and at the right dose

  • Your condition is being monitored adequately

  • Any side effects or interactions are identified

  • Required authority scripts and PBS approvals are up to date

Some repeat prescriptions can be managed via a short telehealth consultation. Please contact reception to discuss your specific medication needs.

If you are a new patient transferring from another practice, please bring a list of your current medications and any recent medical letters to your first appointment so your GP can review and continue your care without interruption.

Mental health treatment plans

If you are experiencing anxiety, depression, or another mental health condition, your GP can prepare a Mental Health Treatment Plan (MHTP). This plan makes you eligible for Medicare rebates on up to 10 individual psychology sessions per calendar year with a registered psychologist.

To access a Mental Health Treatment Plan you will need a longer appointment. Your GP will discuss your symptoms, assess their impact on your daily life, set treatment goals, and refer you to an appropriate psychologist or mental health professional.

Reviews of the plan are required at set intervals to access the full allocation of sessions, so it is important to keep your follow-up appointments with your GP throughout the year.

Mental health concerns are taken seriously at our clinic. You do not need to be in crisis to seek help. If you have been struggling with your mood, sleep, anxiety, relationships, or ability to function day to day, please book an appointment. Early intervention makes a significant difference to outcomes.

After-hours mental health support is available via:

  • Lifeline: 13 11 14 (24 hours)

  • Beyond Blue: 1300 22 4636 (24 hours)

  • MindSpot: 1800 61 44 34

WorkCover consultations

  • Initial assessment and certification of work-related injuries

  • Ongoing management and treatment of the injury

  • WorkCover certificates (Certificates of Capacity) completed and updated as required

  • Referrals for physiotherapy, specialist review, and other treatment

  • Coordination with your employer, insurer, and treating team

  • Return-to-work planning and capacity assessment

If you have been injured at work, please let reception know when booking so adequate time is allocated and the appropriate documentation can be prepared.

Please bring your WorkCover claim number, insurer details, and any previous certificates or correspondence to your appointment. WorkCover consultations are billed to your insurer, not to you directly, provided your claim has been accepted.

Fees, Medicare and billing

Medicare rebates

Most of our GP consultations attract a Medicare rebate. The rebate amount depends on the length and type of consultation. Your doctor will use the appropriate Medicare item number for the service provided, and your rebate will be returned to your nominated bank account, usually within 24 hours of the claim being lodged. All Medicare rebates are lodged by staff at YourSkin Clinic so you don’t have any extra homework.

Bulk billing

Bulk billing means your consultation is billed directly to Medicare at no out-of-pocket cost to you. Our clinic bulk bills the following patient groups:

  • Seniors with a valid Pensioner Concession Card

  • DVA (Department of Veterans Affairs) gold and white card holders for eligible services

  • Children under 16 with a valid Medicare card

  • Health Care Card holders

Please provide your concession or DVA card at your first appointment. If you are unsure whether you are eligible for bulk billing, contact reception before your appointment. Please provide any new issues of your card so we can update your records.

DVA patients

DVA gold card holders are covered for all clinically necessary medical services. DVA white card holders are covered for conditions accepted by DVA. Please bring your DVA card to every appointment. Some services may require DVA prior approval. Your GP can assist with DVA referrals and coordinate your care appropriately.

Private billing

For patients not eligible for bulk billing, a gap fee applies. The out-of-pocket amount varies depending on the consultation type and length. Payment is required on the day. Please ask reception for our current fee schedule at the time of booking so there are no surprises.

Telehealth and after-hours

Different Medicare item numbers apply to telehealth and after-hours consultations, which may affect the rebate and any out-of-pocket cost. Reception will advise you of the applicable fee when booking.

    • Signs of stroke: sudden face drooping, arm weakness, or speech difficulty

    • Severe allergic reaction including throat swelling or collapse

  • Everything discussed in your consultation is strictly confidential and will not be shared without your explicit consent except where required by law. You have the right to access your own medical records at any time. Speak to reception if you would like to request a copy or arrange transfer of your records.

  • Seeing the same GP consistently produces better health outcomes. Your regular doctor builds a detailed understanding of your history, risk factors, preferences, and circumstances over time, and is better placed to detect changes, coordinate your care, and support you through ongoing health issues.

  • Specialist referrals are valid for 12 months for a named specialist or 3 months for an open referral. If your referral expires before you are seen, you can contact our clinic for a renewal.

  • Pathology and imaging results can be as quick as 24 hours or as timely as 1 month. You will always be contacted to discuss your results once they arrive. Please know that being contacted does not mean your findings are abnormal. Telehealth appointments are available for standard findings, however, we recommend face-to-face appointments when discussing complicated, long, or abnormal results.

Please feel free to ask your doctor or our reception for an estimate on when your results will be available.

  • Medical certificates can only be issued for dates on which you were seen, or for a period your doctor can clinically justify. Please attend in person on the day you need a certificate. Certificates cannot be backdated.

    • Bring your Medicare card, concession card, DVA card, or Health Care Card to every appointment

    • New patients should arrive 10 minutes early to complete registration paperwork

    • Bring a list of your current medications including over-the-counter products, vitamins, and supplements

    • Bring any recent specialist letters, test results, or imaging if you have not been seen at our clinic before

    • If you have multiple concerns, it helps to write them down in order of priority before attending.
